How
to do:
1. Fill in the boxes on the form
with the details of your order. If possible, send a copy of
the documents via fax, ordinary post or as attachments to the
same form.
2. I will contact you as soon as
possible with an estimation of costs and date of delivery.
3. If a subsequent agreement is
reached (verbal or written), the order will be formalised.
